About the team
Our Health and Wellbeing Facilitator team provides a specialist service to help people with learning disabilities take positive steps to manage their physical and mental health and to engage with the healthcare services that are available to them.
How we make a difference People with learning disabilities often experience poorer health outcomes and lower life expectancy than the rest of the population– our service is here to tackle this inequality by providing support designed to meet the specific needs of people who have a learning disability. We can also support people who have a learning disability and are also autistic.
About the role
Working across South Tyneside, you will lead our health projects and line manage our team of Health and Wellbeing Facilitators, the Quality Checker Team and our Commissioning Officer with lived experience of learning disability.
You will work closely with colleagues from the Commissioning team, South Tyneside’s Physical Health Hub, and Primary Care Mental Health Team to support people in South Tyneside with learning disabilities to meet their wellbeing and physical health needs.
You will also help people to access the health services and support they need by ensuring people have information that is accessible to them, by supporting services to make reasonable adjustments, and by supporting the delivery of treatment and assessment programmes under the direction of practitioners.
