Care and Treatment Review meetings in South Tyneside

Your Voice Counts is commissioned by South Tyneside Council to organise Care and Treatment Review (CTR) meetings for people with a learning disability and/or autism in South Tyneside.
The meetings are for people who are experiencing or are at risk of experiencing crisis and aim to prevent urgent hospital admissions and sudden placement breakdowns or breakdowns to current support plans.
Someone might need a CTR if they are at imminent risk of hospital admission to establish whether individual care and treatment needs could be met effectively and safely in the community. Other reasons might include a clear deterioration in mental health, or a placement breakdown.

If anyone you are working with needs a Care and Treatment Review (CTR) please complete the CTR CETR MDT+ Review Request Form and send it to yvc.uk@nhs.net.

If you are not sure if you need a CTR and are worried about a person with a learning disability and/or autism in your care, please complete the Crisis Intervention Trigger Document and send it to yvc.uk@nhs.net. This will help us work with the Commissioning Team at South Tyneside Council to identify appropriate next steps. You can also use the Easy Read version of the Crisis Intervention Trigger Document.
